Fibromyalgia and Duloxetine

Duloxetine is a nerve pain medication and antidepressant. It can help treat depression, anxiety, fibromyalgia, diabetic peripheral neuropathy, and other conditions related to chronic muscle or bone pain. Common brand names include Cymbalta and Irenka. However, the oral capsule can be available for both a generic and brand-name drug.

This drug does have a Black Box Warning, which is a FDA warming that alerts both doctors and patients of dangerous effects. It is possible that this drug can make your depression worse. Please reach out to your doctor if you have any questions or concerns.

What is Duloxetine?

Duloxetine is a prescription drug, and it can be used as an oral capsule. It can help treat anxiety, depressive disorder, nerve pain, fibromyalgia pain, and chronic muscle and joint pain. Duloxetine belongs to a class of drugs called serotonin-norepinephrine reuptake inhibitors (SNRIs). The drug acts by balancing the chemicals within the brain that can cause anxiety and depression. Because pain signals are interpreted in the brain, the drug also works to inhibit these pain signals through the balancing of these chemicals.

Taking Duloxetine

It is important to follow your doctor’s directions while taking the drug. Only take as directed, especially because it can be a long-term medication. Depending on what your condition is, how you first react to the drug, and the severity of your condition, your doctor can recommend a different dosage.

Fibromyalgia: Adults with fibromyalgia who are 18 years and older tend to have a starting dosage of 30 mg per day for one week. Then, it goes onto 30 to 60 mg per day.

Major Depressive Disorder: Adults can receive 30 to 60 mg per day at first. Then, the maintained dosage can be between 40 to 60 mg per day.

Generalized Anxiety Disorder: Similar to major depressive disorder, generalized anxiety disorder can start with a dosage of 30 to 60 mg per day. Then, it is usually given as 60 mg per day. Children between the ages of 7 and 17 years old and adults 65 years and older can take this drug. However, they are given a lower dosage (about 30 to 60 mg).

Nerve Pain (Diabetes): Adults are given a 60 mg dosage per day.

Chronic Muscle and Joint Pain: Adults begin with 30 mg per day for one week, and then they tend to take 60 mg per day afterwards.

Important Information: Warnings and Side Effects

There are some concerns and warnings that can arise from duloxetine. Let’s first examine the warnings of duloxetine and precautions you need to be aware of.

Warnings

  • Alcohol: Do not consume alcohol while taking duloxetine. It can increase the risk of severe liver injury.
  • Allergy: Be aware that the drug can cause a severe allergic reaction. Call 911 or go to the nearest emergency room immediately if you believe you may be having a severe allergic reaction.
  • Liver Disease: Do not take this drug if you have any medical or health conditions associated with the liver, such as chronic liver disease or cirrhosis of the liver.
  • Kidney Disease: Avoid taking duloxetine if you have severe kidney disease. Your kidneys can have a difficult time responding to the drug and can lead to an increased risk of side effects.
  • Pregnant: Women who are pregnant should not take this drug. If you are pregnant or plan on becoming pregnant, contact your doctor for more information.
  • Seniors: Adults 65 years and older can be at a greater risk for side effects, such as blood pressure changes. You can also experience low sodium in the blood.
  • Children: This drug should not be given to children younger than 7 years old. Proceed with caution if you are under the age of 18 and seek the advice of your doctor.

  • Fatigue
  • Dizziness
  • Difficulty making decisions
  • Difficulty thinking clearly
  • Nausea
  • Dry mouth
  • Constipation
  • Loss of appetite
  • Decreased weight
  • Increased sweating
  • Stomach pain
  • Liver damage: Symptoms can include itching, dark-colored urine, yellow skin, dizziness, fainting, or pain in the right side of the upper abdomen.
  • Serotonin syndrome: Symptoms can include agitation, hallucinations, coma, racing heart, high or low blood pressure, dizziness, muscle twitching, abnormal bleeding, vomiting, or seizures.
  • Skin reactions: Severe skin reactions can have symptoms like skin blisters, sores in the mouth, hives, or rash.

If you are experiencing severe side effects contact your doctor as soon as possible.

Science and religion have a tenuous relationship in real life and it doesn’t get any better in fiction.

When a murdered young man is found tied to a cross, the suspect list is wide open. A religious fanatic? Crazed serial killer? Unpaid drug dealers?

DI Darren Swift—leading his first major case—and DC Colette Quinn are two sharp investigators that put together a stellar team and dig deep to break the case, but it’s the unlikely pairing of a conflicted, Calvinist nun and a jaded, cynical, foul-mouthed, metal musician that keeps this read moving.

Even before Helen Hope’s tragic history was revealed, I liked her. Pragmatic and practical, the eschatologist was able to see a bigger picture than her life as a nun and question her own actions and judgment.

And the woman thought nothing of taking a risk.

After Swift and Quinn question Helen about the religious implications of the murder site, Helen searches on her own and finds startling similarities between what the police told her about the crime scene and Total Depravity, a Norwegian metal band led by Mikko Kristensen.

When Helen and Mikko team up to find answers, each of their belief systems is challenged leaving both to wonder if they know themselves at all.

Swift is intent on following a drug angle in the murder case, but Helen finds someone is mixing the concept of predestination with junk DNA and genetic markings—does it get any better than that?

Reprobation is spot on with religious detail and the hard science of gene therapy without being high-brow and boring. Characters are well-developed and memorable and will put you firmly on the side of good…or evil. Mikko is my favorite character because he will say anything to anyone. Not for shock value–okay, mostly not for shock value–but, because that’s who he is.

Plot twists are well-planned, perfectly timed… and foreshadowed. Pay attention!

As much as I enjoyed this read and the details, it loses a star because, for me, there were times it was too detailed.

Most chapters end with a situation, plight, or dilemma that had me in a hurry to get to the next page only to be met by deep, detailed descriptions of the locale and/or the historical references connected to it before getting back to the story. Extremely frustrating and took me out of the story each time.

Still, despite that, this is a smart read with a fresh plot that will challenge readers on several levels.

The ending is satisfying but wide and vague, leaving the door open for another adventure with Helen and Mikko?

I hope so, and soon!

Catherine Fearns Author Bio – Catherine Fearns is a writer from Liverpool, UK. Her first novel, a crime thriller called Reprobation, was published by Crooked Cat Books in October 2018. As a music journalist, Catherine is a regular contributor to Pure Grain Audio, and she has also published numerous pieces of short fiction and non-fiction. Catherine has a degree from Oxford University and a Masters from the London School of Economics. She began her career as a financial analyst, but after having four children she retrained as a breastfeeding counselor. Having lived in several countries, she recently moved to Switzerland, where she discovered her love of writing and is a member of the Geneva Writers’ Group. Giving You the Best That I Got was Anita Baker’s biggest hit, scoring #1 on both the Adult Contemporary and R&B charts, in addition to #3 on the Hot 100. A broad crossover song, it became a favorite of jazz, pop, and “light” radio stations.

The song’s writers (Randy Holland, Skip Scarborough and Baker) won the Grammy Award for Best Rhythm & Blues Song for their work on the track. Baker also won Best R&B Vocal Performance, Female for both the song and album. The album also took three Soul Train Music Awards and made Billboard’s #1 album spot for December 24, 1988 through January 20, 1989. That breaks in between (prepare for mood dissonance) U2’s Rattle and Hum and Bobby Brown’s Don’t Be Cruel.

Giving You the Best That I Got is about unconditional devotion to your mate, but according to its co-writer Randy Holland, it didn’t start out that way. Holland says that he wrote the song after moving from Bartlesville, Oklahoma to Los Angeles, where he pursued a career in music. He was “giving it the best that he got,” but things weren’t working out for him in the city. He later changed the song to be about his wife, Jennifer, and their special bond.
